How long will I need physical therapy?
The number of visits needed varies for every patient and condition. At Osteopractic Physical Therapy of the Carolinas (OPTC) in Fort Mill, SC, we determine this only after a thorough one-on-one initial evaluation. Because OPTC provides extended 60-minute sessions with advanced manual therapy techniques, many patients achieve results in fewer visits compared to traditional insurance-based physical therapy clinics. Our goal is always to help you reach your goals in the fewest visits possible, without unnecessary appointments.
When should I start seeing improvement?
Most patients notice meaningful changes after each visit, with early improvements often occurring within the first few sessions. Dr. Zacharko utilizes evidence-based manual therapy, spinal and extremity manipulation, and dry needling techniques designed to create faster, longer-lasting results. Progress depends on the nature of your injury, how long symptoms have been present, and your personal goals.

Do I need a physician referral for physical therapy?
No. In North Carolina and South Carolina, you do not need a physician referral to see a physical therapist. You can schedule an evaluation directly with Dr. Zacharko at OPTC and begin care right away.
What is manual therapy?
Manual therapy is a specialized area of physical therapy focused on skilled, hands-on treatment of musculoskeletal and neuromuscular conditions. This includes joint mobilization and manipulation, soft tissue techniques, and dry needling. Manual therapists undergo extensive post-graduate training beyond standard physical therapy education. OPTC specializes in advanced manual therapy techniques that are grounded in the latest evidence-based practice.
Why doesn’t OPTC accept insurance?
OPTC is a 100% private-pay physical therapy practice because it allows for better care, better outcomes, and fewer visits. Insurance-based models often require therapists to see many patients per hour, limit treatment options, and prioritize billing over results.
By not accepting insurance, OPTC is able to:

Can Medicare patients receive treatment at OPTC?
In most cases, Medicare beneficiaries cannot privately pay for services that Medicare would normally cover. However, certain services such as maintenance, wellness, prevention, and fitness-based care may be eligible for private pay. If you are a Medicare beneficiary, please contact OPTC to discuss your specific situation and available options.
What should I bring to my first appointment?
While not required, bringing the following can be helpful:
Previous imaging reports (MRI, X-ray, CT)
Relevant medical records
A list of medications and allergies
Completing intake forms ahead of time allows us to maximize your evaluation and treatment time.
What should I wear to physical therapy?
Wear comfortable, non-restrictive clothing that allows access to the area being treated. Athletic wear is typically ideal.
How long are appointments?
All visits at OPTC are scheduled for 60 minutes of one-on-one care with Dr. Zacharko. Initial evaluations may run slightly longer depending on complexity, but follow-up visits remain a full hour and are billed at a flat rate.
Will you communicate with my physician?
While not required, OPTC is happy to coordinate care with your physician upon request and/or accept and review any medical notes, image reports etc. that your other healthcare providers may want to share with us. Regular progress reports will not be sent.
Will I a physical therapist every visit?
Yes. Every visit is performed by your doctor of physical therapy, and you will never be passed off to assistants or aides. This consistency is a key reason OPTC patients experience better outcomes in fewer visits.
